چکیده

XB Serrated colon polyps are a group of heterogeneous lesions characterized histologically by glandular serration (ie, a saw-toothed folding of colonic crypt epithelium). The World Health Organization has classified serrated lesions into 3 general categories (Table). Historically, serrated polyps were called hyperplastic polyps (HPs) and were thought to have no malignant potential. However, recent data have established that several distinct subtypes of serrated polyps are the precursors of a group of colorectal cancers (CRCs) that exhibit hypermethylation and arise primarily in the proximal colon, the so-called serrated neoplasia pathway, characterized by BRAF mutations and a CpG island methylator phenotype (CIMP). It has been estimated that serrated polyps might be the precursors for approximately 15% to 20% of sporadic CRCs, particularly in the proximal colon, and most CRCs arise through the conventional adenoma pathway characterized by chromosomal instability. Many studies have shown that benign serrated lesions and CIMP-high CRCs share molecular features. In fact, it has been suggested that HPs lead to sessile serrated adenomas/polyps (SSA/Ps) without dysplasia and then dysplasia develops and finally leads to cancer. Moreover, some interval proximal cancers have been attributed to the serrated pathway, emphasizing the need to detect serrated polyps during endoscopic examinations. Therefore, endoscopists should familiarize themselves with detection of these lesions, particularly those that are located in the proximal colon. G&H How common are the different types of serrated polyps?

Updates in small bowel imaging and endoscopy.

The field of gastroenterology has had an abundance of technological advances in recent years, especially in the field of endoscopy. These advances are helpful to clinicians approaching gastrointestinal blood loss, especially in the small bowel. The authors report a case of a man who presented to the emergency department with obscure gastrointestinal blood loss. Results of an esophagogastroduode...

Advances in upper gastrointestinal endoscopy

The rapidly moving technological advances in gastrointestinal endoscopy have enhanced an endoscopist's ability to diagnose and treat lesions within the gastrointestinal tract. The improvement in image quality created by the advent of high-definition and magnification endoscopy, alongside image enhancement, produces images of superb quality and detail that empower the endoscopist to identify imp...

Therapeutic management options for patients with obscure gastrointestinal bleeding.

Obscure gastrointestinal bleeding (OGIB) is one of the most challenging disorders faced by gastroenterologists because of its evasive nature and difficulty in identifying the exact source of bleeding. Recent technological advances such as video capsule endoscopy and small bowel deep enteroscopy have revolutionized the diagnosis and management of patients with OGIB. In this paper, we review the ...

An update on pediatric endoscopy

Advances in endoscopy and anesthesia have enabled gastrointestinal endoscopy for children since 1960. Over the past decades, the number of endoscopies has increased rapidly. As specialized teams of pediatric gastroenterologists, pediatric intensive care physicians and pediatric endoscopy nurses are available in many medical centers, safe and effective procedures have been established. Therefore...

New endoscopy advances to refine adenoma detection rate for colorectal cancer screening: None is the winner

Colorectal cancer (CRC) is the third most common cancer in males and second in females, and globally the fourth cause for cancer death worldwide. Oncological screening of CRC has a major role in the management of the disease and it is mostly performed by colonoscopy. Anyway, effectiveness of endoscopic screening for CRC strictly depends on adequate detection and removal of potentially precancer...
